As a delegated provider for Blue Cross Blue Shield (BCBS) in Southeastern New Mexico, Rio Pecos offers comprehensive Care Coordination services designed to improve health outcomes for eligible members.
Our process begins with a thorough initial assessment, where we gather critical information on the social determinants of health—factors such as housing stability, food security, transportation access, financial resources, and social support—that directly impact a patient’s overall well-being. In addition, we assess medical history, current diagnoses, and behavioral health needs to ensure a holistic approach to care.
Our expertly trained Care Coordinators then develop a personalized Care Plan tailored to each patient’s unique needs and goals. By addressing both medical and non-medical factors, we provide education, connect members with essential resources, and offer ongoing support to help them overcome barriers to care and achieve optimal health.
