Care Coordination Services for BCBS Eligible Members

As a delegated provider for Blue Cross Blue Shield (BCBS) in Southeastern New Mexico, Rio Pecos offers comprehensive OB Care Coordination services designed to improve health outcomes for eligible members.

Our process begins with a thorough initial assessment, where we gather critical information on the social determinants of health—factors such as housing stability, food security, transportation access, financial resources, and social support—that directly impact a patient’s overall well-being. In addition, we assess medical history, current diagnoses, and behavioral health needs to ensure a holistic approach to care.

Our expertly trained Care Coordinators then develop a personalized Care Plan tailored to each patient’s unique needs and goals. By addressing both medical and non-medical factors, we provide education, connect members with essential resources, and offer ongoing support to help them overcome barriers to care and achieve optimal health.

I am the Director of Care Coordination at Rio Pecos Medical Associates, where I lead programs focused on supporting women and families through pregnancy and beyond. I am passionate about helping patients feel connected to resources, supported in their care, and confident in their healthcare journey.

I am currently pursuing my nursing degree with a strong interest in women’s health and maternal care. Improving outcomes for mothers and babies is something that deeply motivates the work I do every day.

Outside of work, I enjoy running, baking, and painting. I love staying active and finding creative outlets, and I bring that same passion and energy into the work I do with my team and the patients we serve.

I am the Care Coordinator Supervisor at Rio Pecos Medical Associates LTD, where I oversee patient coordination and administrative operations that support quality healthcare delivery. In my role, I work closely with patients and providers to help navigate insurance, financial responsibilities, and care planning, ensuring patients receive clear guidance and timely access to the services they need.

I am currently pursuing my nursing degree with a strong interest in family medicine. My goal is to develop a well-rounded skill set that allows me to care for patients across a variety of needs and stages of life. I am passionate about learning and growing within the medical field and aspire to become a versatile healthcare professional who can serve patients in many capacities.

Outside of my professional responsibilities, I enjoy reading and spending quality time with my family and close friends. I value connection, lifelong learning, and bringing empathy into every aspect of both my personal and professional life.

I am a Care Coordinator with Rio Pecos Medical Associates, where I support pregnant and gynecology patients throughout their unique healthcare journeys. I am passionate about helping patients feel supported, connecting them with helpful resources, and being someone they can turn to for guidance along the way. I truly value the opportunity to make a positive difference in each patient’s experience.

Before becoming a Care Coordinator, I worked as an X-ray technologist for several years, where I developed a passion for supporting patients in their healthcare journey.

Outside of work, I enjoy sewing, crafting, and spending time with my family. You can often find me reading a good book or making a fresh loaf of bread. Whether at work or at home, I enjoy creating things that bring comfort, connection, and support to the people around me.

I help lead our care coordination program supporting women through pregnancy and postpartum care. As both a healthcare professional and a mother, I am deeply passionate about helping women feel supported, informed, and confident throughout their healthcare journey. Being able to connect patients with the resources and care they need is incredibly meaningful to me, and I love being part of work that helps mothers and babies thrive.

I studied secondary education in college and was part of the Arts Leadership Program, experiences that shaped my love for teaching, guiding others, and advocating for people in meaningful ways. Care coordination allows me to combine that passion for helping others with my commitment to improving maternal health and patient support.

Outside of work, I enjoy weightlifting, long walks, cooking any sort of new food, realism painting, reading, and writing. I love staying active and creative, and I cherish spending time with my toddler and husband, who both inspire so much of the purpose behind the work I do each day.

In my role, I work closely with members to help them navigate the healthcare system, connect them to services, and provide compassionate support throughout their care journey. Alongside my work in care coordination, I am currently studying Diagnostic Medical Sonography. I am passionate about expanding my clinical knowledge so I can continue to grow in the healthcare field and provide high-quality, patient-centered care. Outside of work I love activities like hiking, runs, picnics, and discovering different types of cuisines. I love making new connections and doing a bit more when it comes to helping patients as well as my team.

Outside of work, Sofia enjoys spending time with family and friends, traveling, and exploring new places. She is passionate about health, wellness, and helping others feel their best both physically and mentally.

Hi, I’m Trinity Andrew, a Care Coordinator at Rio Pecos. I love connecting with patients on a deeper level and truly care about supporting you through whatever stage you may be in—whether that’s pregnancy or other women’s health needs. My role is to help gather helpful resources, answer questions, and make sure you feel supported along the way. I like to think of myself as your friend first, and then your personal assistant or accountability partner—whatever you need, I’m here to help.

As a wife and mother, caring for families and supporting women during important seasons of life deeply motivates the work I do every day.

Outside of work, I love anything that involves the outdoors or spending quality time with my family, including fishing, hiking, and camping. You can often find me playing with my joyful two-year-old daughter in princess dresses and soaking up the simple, beautiful moments of everyday life—or maybe even playing the occasional video game.

I am a Care Coordinator at Rio Pecos Medical Associates, where I support women and families throughout pregnancy and the postpartum period. I am passionate about helping others and feel grateful to bring that passion into my daily work, providing guidance, education, and compassionate support to the members I serve.

My background in customer care, along with my experience as a Certified Nursing Assistant and as a barber, has strengthened my ability to connect with people from all walks of life. These roles have helped me develop strong communication skills, empathy, and a commitment to delivering quality care and service.

Outside of work, I enjoy embracing life as a mom and spending quality time with my family. We love staying active together, whether that’s going on walks or simply unwinding at home with our two dogs and two cats.

Outside of school, I’m very family-oriented and love spending time with my loved ones. Whether it’s going on walks, doing fun activities together, or just enjoying the little moments, being around family is what keeps me grounded and happy. I’m also someone who values balance, staying active, taking care of my well-being, and keeping a positive mindset. I enjoy being part of communities where people support each other, share experiences, and grow together. I’m excited to connect, learn from others, and contribute in any way I can!

I am a Care Coordinator with Rio Pecos Medical Associates, where I have spent the past eight months supporting OB-GYN patients as they navigate pregnancy and postpartum. I am passionate about helping patients connect with resources, find answers to their questions, and feel supported during such an important time in their lives. Sometimes the most meaningful part of my role is simply being there to listen and provide encouragement when it’s needed most.

I recently graduated from massage therapy school and will soon be taking my exam to become a Licensed Massage Therapist (LMT). I’m excited to expand my ability to help others through therapeutic care and wellness. Outside of work, my life revolves around my family. I enjoy spending time with my husband and our four children, whether it’s at home or cheering from the sidelines. I am also actively involved in my community as a high school cheer coach and by coaching my son’s youth basketball team alongside my husband. Supporting others, whether in healthcare or everyday life, is truly what motivates me, and I’m grateful for every opportunity to make a positive impact.
